The Directory update form is for updating CSUSB faculty and/or staff information only. We do not publish "Student Assistant" information. All requests will be verified with the selected Department before they are processed.

Please note that submitting a directory update cannot change billing information. If you are moving an extension to a different department, a work order must be submitted to the Telecommunications Office at ext. 75133 or through the Work Order Request Form in order to make the change.

Removing an individual name by using this form, will result in the name being removed from the extension record. However, the extension will remain in the system with all associated charges.
